Supporting Collaboration for Health: 2025 Trailblazer Planning Grant Awardees

The Trailblazer Planning Grant, awarded by Indiana CTSI Community Health Partnerships, supports teams of university researchers and community-based organizations working together to improve the health of Indiana residents. Rather than funding pilot projects, the grant is designed to strengthen and develop meaningful community-university partnerships. Funding supports planning and partnership-building activities that help teams prepare competitive proposals for future funding opportunities, such as the Trailblazer Award or other external grants. Awards are up to $5,000 over a period of up to two years.
In December, seven teams of community and university partners were awarded the Trailblazer Planning Grant to strengthen and advance their collaborative work. Together, these partnerships are addressing some of Indiana’s most pressing health challenges, including cardiovascular disease, child exploitation, mental health, and gun violence.
