Connections IN Health is a partnership with the Indiana CTSI, Indiana Department of Health (IDOH), and IU Simon Comprehensive Cancer Center.

Through three coalitions focused on chronic health coalitions, the group strives to make a positive impact on health outcomes throughout the state.

The Coalitions

Cardiovascular & Diabetes Coalition of Indiana (CADI)

Engaging partners across the state to address the burden of cardiovascular disease, diabetes, and stroke through education, collaboration, awareness, advocacy, and access to care.

Hoosier Health & Wellness Alliance (HHWA)

Addressing food security, accessibility, environmental issues, and other root causes of obesity and lack of physical activity.

The Indiana Joint Asthma Coalition (InJAC)

Improving the health of Indiana residents with asthma through community collaboration, reduction of environmental triggers, and increased public awareness of asthma as a serious chronic disease.

Indianapolis Health Equity Access Outreach & Treatment (iHEART)

Connections IN Health is excited to partner with the iHEART collaborative, delivering interventions embedded in clinical settings and community institutions including barbershops/salons and local places of worship—directly bringing health resources to those facing barriers to care.
