County Engagement
The County Engagement project addresses health challenges in Indiana counties. The goal of the project is to positively impact the outcome of an identified health challenge present within the community through the sharing of resources and evidence-based interventions.
Connections IN Health Engages With Counties By:
- Surveying community members and hosting listening sessions so community members can share their perspectives on successes and challenges related to chronic disease within the county.
- Compiling and analyzing data, and then sharing that information with with the community.
- Identifying selecting of a specific and actionable health challenge within the community.
- Sharing resources, implementing evidence-based interventions, conducting community-readiness assessments, and expanding the community’s network with coalition experts.
Connections IN Health originally identified 10 counties to begin in using county health data and factoring in social determinants of health. The project continues to expand, with an 11th county being added in 2024. Their work is bolstered by building and partnering with coalitions in the counties and the counties’ interest in partnering on a project.
